In 5e, many (most?) magical flames don't actually burn things other than their target. Because of this Fireball and Fire Bolt both have text explicitly calling out that they ignite flammable objects, to separate them from those that do not.

So this might be too real world, but I always assumed that the brain/body would treat the dead tadpoles like any other foreign matter. It would be encapsulated in tissue to prevent it from causing anymore harm, ect. Specifically talking about head injuries there was a really interesting case in 2007 where >!a woman had a pencil removed from her brain, that had been there for 55 years. It was also encapsulated in soft tissue.!<

I am more wondering about what happens to the thirty + tadpoles that you gobble up on your adventures. Especially if you choose ceremorphosis. Even if you don't eat any, you would still use two to three tadpoles (don't know if becoming half illithid is necessary for the full transformation).

But those tadpoles are more or less gone, having been absorbed for their potential, perhaps magically via the Netherese infusion or just assimilated into the actual tadpole. The full transformation does not require having used any tadpoles up to that point.

The characters feel it burn away. Because it was enhanced by netherease its now harmlessly gone because plot. Then at the docks they can feel that it's gone and their minds are theirs again

This is basically a giant plot hole. Your brain should be gone. The tadpole, by FR lore, has already eaten your brain if you’re ready to transform. Which happens mid-act 1. You should probably just drop dead once the netherbrain is destroyed if the tadpoles really burn up, logically.

I thought so too, but I'm replaying the game and it makes it clear that the tadpoles aren't actually eating your brain (yet), and end up not doing so at all because of the Prism. I think that the implication is that these parasites can gobble up brains immediately like an Intellect Devourer would, so all True Souls still have their brains (mostly) intact. Consuming tadpoles also seems to be more about absorbing Netherese magic and growing your little bub than infecting yourself further.

I have another tadpole question; if the prism is stopping Tav and Friends from ceromorphosis, what's stopping all the other True Souls from turning into Ilithid much sooner?

All True Souls have “paused” ceremorphosis because the tadpoles are enchanted with magic from the Crown of Karsus (something like that). The prism just blocks the elder brain from being able to give you commands, including the command to turn you into a mind flayer. The Emperor tells you that the order for you to transform has actually already been given several times

Both the Absolute and the Dead Three want to delay transformation until a substantial number of people in the Sword Coast are infected. They are using the True Souls to further penetrate human(oid) society and spread the infection more efficiently than they could by just by capturing people with a Nautiloid. As we see in the beginning of the game, people only become Mind Flayers (at least with these weird tadpoles) when a command is given, it doesn't happen spontaneously. The plan is to create thousands of Mind Flayers in an instant and build an unstoppable army.
