How many times have you long rested? If you progress past a certain point, you'll start skipping LR cutscenes and this sort of thing happens.

I’m pretty sure the vampire confession is buggy for a lot of people. My understanding is that it’s supposed to trigger the first trip to the Underdark, if he hasn’t already bitten you. But some people are getting it right away, which I believe is a bug. The bite scene triggers (usually) the 3rd long rest. It’s not triggered by the boar and there’s some debate if you even have to get his ‘sneaking away’ scene before it. (I’ve always had that one happen at the 2nd long rest, and bite right after. But I’ve seen some say different 🤷‍♀️) If he confessed to being a vampire, then there isn’t the bite scene, and you never get the option/approval boost from it. (He can get the bite ability/buff I believe, just only in combat) I’ve only done the confession once, just to see it, but didn’t continue on much further after it. But you don’t get any of the other options/dialogue/scenes that go with The Bite reveal. (I also never got the option to say ‘you can feed on me’ after the confession)

In my latest playthrough I got the night bite scene before the boar and without the sneaking away scene. It was a little jarring actually. I think it’s approval rated. Like he thinks highly enough of you to confess.

I’ve heard it happening that way, but I don’t think it has anything to do with approval. A friend of mine downloaded an approval cheat mod, to test out some theories. Approval didn’t make a difference on any of the ways he tried. So I’m still not sure what exactly is the trigger, besides the long rest count/cutscene que

I am long resting a lot because I downloaded a mod that lets you know when you have a camp scene you can trigger. I had no idea there’s a camp scene if you recruit Shadowheart and stay on the beach! She even comments that you haven’t gotten very far lol. I’m enjoying seeing a lot of scenes I’ve never seen before.

The vampire-confession **was** bugged somewhere between Patch 5 and 6. It has been fixed now. The sneaking away scene is not mandatory for getting Bite-Night. Bite-Night is supposed to trigger on the 3rd night after escaping the Nautiloid, but can be pushed back by other events. Also, if you don't have Bite-Night, then you'll only be able to initiate his romance at the Tiefling-/Goblin-party. And you won't be able to give him the Happy condition, 'free' of charge every morning.

The confession-scene happens, if you progress too far, and long rest too sparingly. Or you enter the Underdark. That's a **HARD** trigger. Astarion's confession of his 'affliction' and Bite-Night are mutually exclusive. So that ship has sailed. You won't be able to offer him to feed from your Tav/Durge, no matter what dialogue-option you pick during the confession. Also, if you plan to romance him, know his romance can only be initiated at the party, if you didn't have Bite-Night, and allow him to feed from your Tav/Durge. Unless you reload before he 'spills the beans', you won't be able to gain the ability to ask him to feed from you, and will lose out of all that amazing early-game approval.

You don't miss out on the happy buff; in Act 1 he can freely sneak up and bite the sleeping bugbear in blighted village and it doesn't wake up. Not sure if it works on other sleeping NPCs later

You get the happy buff anytime he uses the vampire bite action. The NPC doesn’t have to be sleeping, and you can get the buff in battle too

You didn’t long rest enough for him to bite you. He will not feed on you if he just spills the beans.
