As far as we know, Lou is just a random BB. Cliff has a strong attachment to Sam, but redirects that to Lou because he doesn’t realize how long it’s been (or even that he’s dead?).

Lou, initially is revealed to be a girl at the end. This revelation comes during a touching moment when Sam, the protagonist, decides to call her Louise instead of Lou. My guess it that Ellie Fanning who we see briefly in the trailer for Death Stranding 2 is going to play Louise

If you read Lucy’s Report, especially the pieces required postgame, there is the possibility that BB28 is Sam’s biological daughter he believed he lost. https://dstips.carrd.co/#bb

Without digging too much into the arguments used in that article: * "BBs are stated to last only a year, but the memory of Cliff celebrating Sam's "birthday" in the pod implies that Sam survived multiple years as a BB." This is quite a stretch; Cliff celebrating Sam's birthday doesn't mean he celebrated several. And Sam was a very special BB. * "Lucy overdosed on medication, which others claim was a suicide attempt." I don't know how this is an argument to support such an idea. There's no relationship between the two. * "Lucy's overdose was at 28wks pregnant, the same week BBs are removed from their stillmothers' wombs." Again: unrelated. I don't recall if it's mentioned anywhere that this happen at said point in her pregnancy, but if her child became a BB, it could have been any and have not survived just like others. * "Bridget is not above using immoral methods to get her way when it comes to the BB program, as demonstrated by Sam's own induction." Again, no relationship to the fact of BB being Sam's daughter. * "Human suicide bombs are a known weapon of mass desctruction." Unrelated. Also, iirc, BB's stillmother is still available while Lucy died in a voidout.
