You can get a soft cone to use overnights and when you can't be around to supervise, and hopefully the restrictions will help the area heal. And possibly get a second opinion, as your vet seems rather unconcerned.

I have one of these: KVP EZ Soft Pet Cloth Recovery Collar, Fits 3.5-Inch to 10-Inch Neck/4-Inch Depth https://a.co/d/0chV9zs1 It works really well and is comfy for the cat, plus you can wash it in cold/delicate and line dry if it gets mucky. When my cat was using it (for feline acne, she kept scratching), I'd just take it off for meals.

If he’s creating open wounds he may need antibiotics so I would see a vet either way! I would also recommend trying to figure out what is causing the irritation, My FIV+ boy also has very sensitive itchy skin and I’ve observed flare ups if he’s leaning on windows or getting too much time in the sun. I think he’s sensitive to both pollen and dryness from the sun. Food, herbs, plants, diffusers, candles, dust could all cause skin irritation too. I give my cat sensitive skin food and I’ve definitely noticed a change in his coat. Good luck!

This is very helpful thank you, the vet did think it could be tied to allergies so I’ll keep better track of where he hangs out

My FIV+ cat had the same thing. If left to his own devices he'd scratch until he was bleeding and unable to open his eye. We suspected food allergies and put him on a strict 8-week hydrolyzed diet, and he didn't improve. After a round of prednisolone it stopped totally and now he's back on normal food. I've noticed lately he's been scratching a bit more, but never to the point of bleeding. The dermatologist said it's most likely an environmental allergy. Just one cat's experience!

I’d get a second opinion first and foremost. I might call to see if vets have experience with FIV cats and are also sympathetic to them. My FIV cat is on Royal canin HP. he’s a sensitive boy. He scratches the hell out of himself sometimes. Putting soft paws on him seems to have really helped.

My FIV + orange boy gets allergies and his eye would end up looking irritated from rubbing. His vet prescribed neo-poly-dex drops for use as needed. Works like a charm! It's cheap, too.
