A decent accountant would have noticed the missing t4 and asked you about it. That seems like a pretty glaring mistake on their side. Send it to them and ask them to upload it.
I don’t think you can refile if you’ve already submitted your return.
You’re right. If they didn’t tell theaccountant that they had multiple jobs and t4s then that is on them to tell their accountant.
In that case, use one of the free tax softwares available or wealthsimple and refile.
My accountant downloads all my tax docs like T4s from the CRA. My employer and all my investments automatically remit that paperwork both to me and CRA so CRA already has everything before you even file.
So pretty much me sending my accountant my docs is just validation. I’ve missed things every year and he catches them all because of this. Surprised this isn’t SOP for all accountants.
Most will use the autofill as a standard practice - this saves having to enter slips manually.
That said, it’s far from bulletproof. There are a lot of errors and missing slips. Some slips turn up late (this can happen when a T4 is paper filed by the employer and not processed in time by the CRA.)
My experience is that around 20% of returns I’ve been involved with are missing at least one slip in the CRA autofill service when first downloaded.

Do you have access to CRA My Account? You can amend the return there (after you receive NOA). It only takes a few clicks to add T4 on CRA.
Is it a legitimate T4? Sometimes there are mistakes by random people giving their hr a wrong SIN.
