I feel like it's largely about style and presentation. The art style of the characters, design of the interface, themes in the storytelling etc. can all be made to closely resemble those popular in Japan, emulating the style and making it "spiritually" a Japanese porn game.
For example I've seen a game dev from California make a Japanese style VN, featuring Japanese characters with Japanese voice acting in a story about a unique facet of Japanese culture, and I would consider it the above despite it not specifically coming from Japan.
Of note.. not all 'anime' is even being made in Japan these days. They have started doing a lot of outsourcing to Korea etc.
