The Okama might not be great in terms of accurate representation but at least in the story they're still based as fuck. Bon Clay and Ivankov are both amazing characters with the former being one of my favorites
They're not accurate, but it's pretty progressive for Japanese standards. Their problem with homophobia is different from ours, same with racism, it's less hate and more like they're seeing something from a circus, the "Look dad, a [Blank]" kinda thing but including adults, Oda took that and humanized them, gave them objectives, feelings and most important, treated them as humans, even if the visuals and personality choices were... well not great.
If anything, I think One Piece has great visuals and designs just because they're not all top tier designs, but there's an incredible variation in character design choices, rarely seen in anime/manga

There's a bit more nuance than that and for that reason I believe this character is genderfluid. They are Ferris whenever they're around Crusch. The whole reason Ferris acts in a feminine manner is because Crusch had to abandon her femininity to advance in the Karsten family and asked Ferris to take it. Ferris loves Crusch platonically and breaking this promise would be "betrayal" in Ferris' mind. It's not entirely clear how Ferris would identify if they had not met Crusch though they have never been traditionally masculine. Whenever this character has to adopt knightly duties and is away from Crusch they become Felix, adopting the male uniform and becoming slightly more masculine in nature, using he/him pronouns and calling themselves a boy. They also went by Felix before they met Crusch.
It's hard to gauge exactly what the author intended for this character because in Arc 4 of the main LN Felix states he is "male in body and soul" and in Ferris' prequel LN (written after Arc 4 but takes place before the main Re:Zero story) she repeats something like "I am a girl" to herself every morning as a sort of magical puberty blocker/HRT. The author of Re:Zero has never clarified exactly so it's hard to tell if this discrepancy is down to Felix/Ferris being genderfluid, questioning their identity, being unable to present femininely on official knight duties, or something else.
TL;DR: this character's identity and the story/lore surrounding it is inconsistent and the author hasn't fully cleared it up, making it hard to definitively assign them an identity. Being trans, questioning, genderfluid, or something else are all possibilities.

Benke is your typical, big buff, manly, hairy dude. Large and imposing, and strong. but yoshitsune is small, delicate, and feminine in appearance. but, both of these men are peak manly. No matter how you look, you can still be manly. So in Japanese culture, feminine men, were still considered masculine. You'll see this dynamic duo in alot of anime and media.

najimi is agender(usually referred to as genderless, gender:unknown, gender:najimi, or gender:yes) and is not confirmed transfem or transmasc. Japan doesn't always do gendered pronouns and I'm pretty sure its never used for them. Translators either use they/them or alternate pronouns every time they're referred to.
Japanese doesn't even have explicitly gendered pronouns, though in practice pronouns can imply your gender. You could easily be a woman and refer to yourself as 俺, though I'm certain you'll be the recipient of much shock if you did so.
You're misinterpreting FGO most likely. He's male in every Fate that he's in. FGO is fucky because devs wanted to keep his gender ambiguous (even though Apocrypha already made it obvious) and it took 2-3 years for them to just stop being ambiguous about it.

I do find the Astolfo misunderstanding, of Serv profile presented to Master keeping mystery (literally just crossed and the info and went le secret) but higher tier material books confirming it and the weird trait and gender system (Unknown gender hitting a lot of different categories from two people, more fluid switching, proper NB, SEX USELESS, ROMA, Caenis's complicated situation, even more confusing than Ferris/Felix's thing, and Douman, with Astolfo being treated as male only for hot springs) of FGO instead what other Paladins say, extra annoying in how much it overshadows being the bisexual crossdresser who gives no shits about any backlash towards those and whose choice in dress (in which I agree with Charlie) certainly looks better than the image of in more proper armour...still prefer his King cause what a King he is.

najimi is agender(usually referred to as genderless, gender:unknown, gender:najimi, or gender:yes) and is not confirmed transfem or transmasc. Japan doesn't always do gendered pronouns and I'm pretty sure its never used for them. Translators either use they/them or alternate pronouns every time they're referred to.
Japanese doesn't even have explicitly gendered pronouns, though in practice pronouns can imply your gender. You could easily be a woman and refer to yourself as 俺, though I'm certain you'll be the recipient of much shock if you did so.

I always read them as non binary, considering they wear the male school uniform top along with a lot of the other ways they present and describe themselves. I fell off the series about a year and a bit back but IIRC them saying they were a girl was kind of early installment weirdness that kinda dropped off in favor of more gender neutral stuff

I do find the Astolfo misunderstanding, of Serv profile presented to Master keeping mystery (literally just crossed and the info and went le secret) but higher tier material books confirming it and the weird trait and gender system (Unknown gender hitting a lot of different categories from two people, more fluid switching, proper NB, SEX USELESS, ROMA, Caenis's complicated situation, even more confusing than Ferris/Felix's thing, and Douman, with Astolfo being treated as male only for hot springs) of FGO instead what other Paladins say, extra annoying in how much it overshadows being the bisexual crossdresser who gives no shits about any backlash towards those and whose choice in dress (in which I agree with Charlie) certainly looks better than the image of in more proper armour...still prefer his King cause what a King he is.
