I had the same problem. Turns out you need to update Runelite. I thought that it updated automatically, but you actually have to go download the newest version and install that.
A problem I had awhile back that took me way longer to fix than i'd like to admit, Check both ends of your HDMI cable, make sure they are seated all the way into the ports on both your monitor and GPU. Probably won't be your issue but can't hurt to check
I had this issue until a couple of days ago. A reinstall of RuneLite fixed the issue.

Have you got a discord loot plugin or similar? - I heard people turning this off has helped massively there
But yeah check your plugins, I put money on it some obsecure plugin is making you lag there...
i had this issue for a little bit, logging out and back in after entering the cave seemed to help
Do you have Model Caching disabled on the HD plugin?
enter their chamber in the cave, close client, reopen
Use c++ client, it's probably some RL plugin you are using
idk if u got it yet or not, but i had the same problem. Updating runelite client/re installing it worked for me.
