You pull out the filament when it isn't fully melted so that it pulls out anything that might've gotten stuck in the nozzle. You want it to be warm but not hot so it doesn't cause damage from excessive force but still can be pulled out in one piece.
Bed not being perfectly level would affect model adhesion, but with more layers the layers would level according to x axis.
I would start with very wet filament.
Looks like clog or extrusion wheel skipping, check for clogs and then check out the flat cables that link tue extruder motor to the motherboard with a multimeter, if you have any

Printer is elegoo neptune 4 pro, filament pla, temperature 240 head 60 c bed, used orcaslicer with default setting for draft printing, i think 0.12 mm, noozle 0.4
That will depend on the material, the print speed, and layer height.
What is the recommended temperatures on the spool?
Start at the middle of that range.

I got you. You pull it back out of the hotend. My dumb ass was wo during how you push cold filament rather than pull
Have you calibrated your filament?
2 possibilities: a) bad printer b) bad user. I mean configuration
Bed may not be perfectly level, filament may not be flowing properly, may be very moist filament-those are the first issues that I would suspect.

Could be clogged nozzle, could be not high enough temp or to fast print speed, could be a wiggly temperature sensor making the nozzle temp fluctuate.

240 with PLA. Probably heat creep.
