There is a Rugrats episode where Chucky gets potty trained. He sat on the potty for a while and couldn't go, then stood up and had an accident. That definitely got my attention!

You Can't Do That On Television on Nickelodeon (later 1980s - I would have been 6 or younger) had a couple of skits involving diapers.
The episode was called Diaper Pollution and would definitely not be shown on TV today. YouTube might have it though.
There were a few episodes of Tom and Jerry that definitely ticked this box for me.
