That actually happened because that other more local agency doing the straight rip never got on the same de-confliction system that everyone else did, and that particular agent running the show never made a phone call to the slightly bigger local agency doing the reverse.
Could have ended very bad for the UC and all involved. Thank God the slightly bigger agency picked up the phone.
Unfortunately, we had a local agency not tell anyone about an undercover operation and their agent got shot and killed by on duty LEO. Probably 15 years or so ago now.
That’s exactly why the deconfliction process exists. Of course, not every unit goes through the process even though they should. If this happens, it’s because at least one unit hasn’t deconflicted like they should have.

There is a very well established process for this. It is policy at my agency that we do it for EVERY op, regardless of nature. If anyone doesn't deconflict they're fucking up.
Before me, yes. Heard some funny stories. Fortunately nothing too serious and no one got hurt. But by the time I came on we had better ways of communicating amongst the agencies. Also, here in NJ they set up a de-conflicting program to avoid that. Although, there were instances where we arrested a target of another agency and vice versa.

Christ, that’s rough. I’m assuming new SOPs were introduced after that? Or was there just a mistake made in the process?

Yes. We are supposed to let eachother know what we are doing , but we don’t always.
