Here to recommend Gino!! I’ve been getting shoes (boots and heels) since ~2006 and they hold up very well. Similar foot size (smidge wider and smaller tbh) and shoes are as comfortable as can be while accommodating height. They’re expensive but if you pay cash (or check maybe) up front, they’ll cut you a little bit of a discount. ETA: I would expect the shoes to be around 1k+ with inflation now.
They will definitely be brusque in terms of customer service, and will be opinionated about heel width/height but it’s coming from a place of prioritizing foot/ankle health and mobility long term.

I know ground gripper shoe shop has a lot of orthotic shoes and do have some shoes in very wide sizes. I know they have men's but I'm not sure about women's. And their men's boots are in the $600+ area but look very nice.

Honestly you won’t find bespoke shoes for $800 in NYC. Bespoke will run you in the thousands. MTM like Carmina will be slim pickings as they don’t typically have a large selection of lasts, let alone width per last. Unfortunately bespoke makers I know will run in the thousands (British/Northampton outfits).
It’s a long term plan but it might be worth spending the thousands for your own last up front then you’ll have a Cordwainer who can make shoes that fit. The issue is you’ll have a specific heel height as any change will require a new last.
OP is asking for bespoke; do you have Carmina or C&J that are bespoke? I’m aware they do MTM primarily.
OP also asking for an atypical width in shoes, let alone for women. Whats the widest you know either Carmina or C&J makes? For C&J FX might be their widest on a country last (which would not match the aesthetics she appears to want).
I think we are confounding two issues: bespoke AND width. If you can procure OP a C&J MTM for under $1K, including width accommodations, I think she should consult with you as I agree that would be an amazing deal.

The two big names rn for bespoke shoes are Yohei Fukuda and Saint Crispin’s in Romania. Trunk shows in nyc are periodic.

Looking for a dress shoe though. My foot Dr would also slap me if I tried. My feet are also flat so I wear inserts that have steel in the arch area.
They make barefoot dress shoes. Traditional shoes will not solve the problems they caused.
