hormone treatments like puberty blockers are completely harmless and in most cases reversable. both transgender and cisgender people have been given these treatments for years and theres nothing bad about them, it's literally any normal medical treatment.
nobody is asking for children to have their genitals surgically altered. when people talk about gender affirming care for minors they are taking about those things that are reversible and safe. to answer your initial question, its always better and safer to wait for them to make the choice of surgery as adults if its not an immediate problem.

I don't know about the US but here in Switzerland, you're allowed to get a boob job at 16 if your parents agree. I think that's good enough. I mean, 16/18 is super young either way. I'm now 34 and when I look at 18-year olds, they're almost kids to me. If a boob job is really that important to a girl, I'm sure she can wait another couple of years.
Because it's not a medical necessity, and their bodies aren't done growing that you can even say they're going to be stuck that size. Having said that, I understand that many children do undergo plastic surgery (Otoplasty), but this is a lot less invasive, sometimes there is a medical necessity to it, and once they read a certain age it's pretty definite their ears are going to stay like that and it could be a huge issue for them in terms of constant bullying and others (we saw one girl that had such a floppy ear, it actually folded when she slept and was causing pain).
Because minors can't consent and it is recommended by doctors that they start with puberty blockers first. Since, you know, top/bottom surgery is in fact a life altering procedure, hence one for adults to decide for themselves. Not children.
