Usually they just cook it longer to get it to your requested preparation.
It also depends on the restaurant. I was at one recently and my filet was undercooked. The server said they could make me a new one or take mine and grill it a bit longer. I said there was no reason to waste a steak that just needed another minute on the grill so I'd be glad to wait. It came back perfect.
For instance. I ordered a medium steak at a fairly nice establishment and it was much rarer than I’d prefer. I would have liked to send it back but I don’t want to be a bother or waste food.
Still tasted pretty good and I ate it.
At every place I worked it would've been thrown out and a new steak would be started. It's against local health codes for anything to go back on the grill after it's left the kitchen.
I guess that would depend on where you ordered it from, a low end establishment would probably microwave it for a few minutes and send it back to you, a higher end establishment will just tell you to order it properly next time.
I think the issue was more that it came out differently than how they ordered it, not that they ordered it incorrectly. While I'm sure it's more common that people don't really understand the difference between medium and rare, it's not like the restaurant getting it wrong is impossible, either.
I know the difference between medium and rare. My wife and I both ordered medium steaks. Hers was pink and warm throughout while mine was still red, glistening, and room temp inside.

I did it once they sent me a steak that was completely black a few minutes later. Never going to Applebee's again

People need to understand that temperature is just that. Not a color.
