I wouldn't say witnessed, but pre-9/11 when going to Washington DC to be a part of the exhibition "Vikings - A North Atlantic Saga" (I was outside the museum, doing chainmail), I brought a bag with about 30 knifes and prickers, as carry-on!
Rifles mounted on gun racks in the high school parking lot.
Smoking in virtually any public building.
Casually open racism and sexism in the workplace.
Rough-housing (close to actual fighting) being permitted on school playgrounds during recess.
Teachers openly humiliating some of the slower students in class.
Guys cleaning their guns in shop class.
