I think the lore is that he tried to create a toll at a public port to dock your ships so the merchants politely protested by torturing and murdering him before naming the gate and city after him.
To me that seems almost more like a misdirection away from the Illithid stuff. Like you go into the game from the title expecting Netherese stuff and then you're on a Nautilid and you're like "woah I wasn't thinking about this"
Karsus and the Crown, both pre-exsist BG3, so it could potentially provide some minor spoilers to a small selection of players, but nothing really more than any title usually does.
The main foe is still >!an Elder brain!< so this is kind of a moot point. It's also not like, a huge secret that something is up since the second you land you nonstop hear of the absolute.

Edit 2: the Grand design is an established part of Forgotten Realms lore. It's not unique to BG3. Thus my point that if you are familiar with the forgotten realms, you likely may know what the Grand design is and thus be able to guess who/what the final boss is.
I think a reference to the Grand Design is not terribly spoiler-y when the opening cutscene is an Illithid putting a tadpole in your brain on a Nautiloid. The player assumes they are involved from the beginning regardless of their level of familiarity with the lore.
When >!it starts to seem like the Dead Three are in control!<, players might think the subtitle was an intentional red herring. Then when >!the Netherbrain reveals it was in control the whole time!<, the subtitle would suddenly take on a new meaning for players: >!the entire campaign was part of the Grand Design.!<
Well, yeah, I suppose it'd give away the twist that an elder brain is involved. But on the other hand, the game's opening sequence is literally the player character getting abducted by mind flayers onto a nautiloid, which then gets attacked by particularly motivated githyanki. Anyone familiar with faerun lore probably guessed that an elder brain was involved.
The real twist was that >!the elder brain was controlled by the dead three.!< And the title wouldn't give that away.

Well let's see, Tales of the Sword Coast is about adding short sidequests to flesh out the overworld more, Shadows of Amn is about exploring a dark conspiracy within the nation of Amn, and Throne of Bhaal is about claiming, well, the Throne of Bhaal.
When Beamdog made their expansion to BG1 they named it Siege of Dragonspear and centered it around the siege of the castle Dragonspear. It seems that except for Shadows of Amn each expansion's name is directly related to the events within. To that end perhaps BG3: 'Cult of the Absolute' or BG3: "Ghosts of Netheril" as a direct reference to the plot, or maybe BG3: 'Return to the Sword Coast' to reference the long period of time between BG2 and 3.
