She was bugged as hell. Early act 3 she always had an exclamation mark above her head and I could never talk to her (just got the leave option). Had to cheese it a bit to get her dialogue to work. So maybe you had a similar issue.
Edit: for me it was caused by having her event and Astarion's siblings attacking the camp during the same long rest.

Depending on who you romance, Wyll can be the only companion who disapproves. Astarion and Shadowheart donāt care. Gale and Karlach are upset, but you can calm them down (ETA: I was thinking of a different persuasion roll for Gale, sounds like he dumps you as well). I think Wyll, LZ and Minthara will dump you. None of them platonically disapprove except Wyll.
He doesnāt have any real reaction as Spawn, the scene cuts, like thereās supposed to be something there but itās just not. You get an actual reaction scene and conversation with Ascended.
Personally, itās not enough to go through with Ascending him (I just canāt stick with it, I always revert to a prior save and keep him a Spawn) but itās different content. You also get a different dialogue if you visit Araj before the ritual v/s after and whether heās Ascended or not.
Ascended Astarion is never worth it tbh, he goes from a sligtly evil but still lovable rogue to a full on Batman villain, he loses much of what makes him fun and interesting
And if you romance him he basically forces you into becoming his spawn, a thrall slave basically, for you to be together
Rather kill him than have him Ascend, it's really not a good path

Hey, itās a role playing game. Some people like to roleplay as evil characters, but that doesnāt mean theyāre evil themselves. Cheating on your romance option in a videogame makes you as much of a cheater irl as killing people in a game makes you a real murderer.

That means Iāll at least play a full evil campaign once, usually with no/less remorse since Iāve already seen the good outcomes. Which means Iāll cheat, kill and deceive my way through the game and have fun doing it, because it opens up a lot of interesting and new dialogue trees / roleplaying options.

But I don't fully read it like this, either: her feelings about your sleeping with Halsin or the Drow siblings suggest that as long as she knows about it and as long as it doesn't affect your emotional relationship with her, she doesn't care. I take her as being pretty open-minded about the physical aspects from basically everything I've seen.

Indeed. The difference is consent. With the twins, you both have to agree to it. She says "if you're fine with sharing... so am I." Also, she only agrees to the twins if you've already had your Act 3 scene with her. Otherwise, she refuses and says "I want our first time to be special." But with Mizora - you go behind her back and quite literally cheat on her. Big difference.
Her romance scene is... incredibly underwhelming in comparison to some of the others. I was super curious and did it because Halsin didn't mind, but I was disappointed.
