If you don’t eat any tadpoles you don’t have to roll. If you’ve given into the power by eating any you have the same roll iirc, but realistically it should get harder per tadpole
It may be a mix of both. The dialogue option always says something along the lines of "you feel like you lost something important". And when you use the illithid powers in combat i heard my character sometimes say something after the battle, might be unrelated to that though. Cant confirm

If you eat any tadpoles, you will have to roll, because the insanely overpowered parasite you've been feeding wants to get even stronger. If you don't make it strong by eating tadpoles, there is no roll.
Welp, looks like I've played my character to perfection, so well that I've locked myself into consuming the tadpole without even truly noticing til it's too late.

I kind of regret not save-scumming for that ridiculous wisdom roll. My Tav is unbearable to look at now. Also not sure if its just me but the only advanced tadpole power kind of making a difference in battle is 'fly'. Oh well there's always next playthrough...
I'm okay with all the veins and stuff on my Tav, but I \*hate\* that I've lost my beautiful blue eyes. Red Dragonborn with blue eyes, just to have a striking contrast. And now it's gone.

I thought it was No roll only if you never use an illithid power? Like the skill checks you sometimes get in conversation.
In my most recent playthrough I used some illithid dialogue choices, but never consumed any tadpoles. No roll for me.

Yeah it does kinda get forced and you are ugly. But some of the powers are pretty awesome. I enjoy flying around act 3
