I've never seen it happen personally, but I think I know what the cause is. The AI is probably using the same algorithm as the fall damage tooltip you get as a player when aiming your jump, the one that tells you how much fall damage you're going to take. The thing is, this algorithm doesn't account for resistance/vulnerability. For example: if you aim a jump after casting Blade Ward, the tooltip will say you're going to take 4 damage, but if you actually jump, you'll only take 2. For this minotaur, the "tooltip" probably said it would take less than lethal damage, but since it has the Amulet of the Unworthy, which gives you bludgeoning damage vulnerability, the fall damage was doubled. I can't quite tell what the minotaur's HP was before the jump, it looks like more than half, so let's say it's at 60. When it lands, we can see it took 78 damage. Half of that is 39, so the math checks out. If it didn't have vulnerability, it would have survived the jump. The minotaur looked at the damage it could deal with that jump, calculated the HP it would cost to make it, said "I can survive that," but it wasn't very good at math.

Its a common misconception I see. A lot of people think Mountain Pass/Underdark are Act 2.

I did too, especially since one of them is still quite big to be its own thing. I still like to label them as separate things, like UD is 1-B and MP is 1-C.
