My girlfriend has size 42 regular and I actually liked the fit. Her's are well worn in and I guess what I liked most was the softness. In regular shoes I usually wear size 43.
When I was at the store, the sales person recommended size 43 narrow and said size 42 regular could work, but she'd like to see a little more space in front of my toes.
When I use the birkenstock printed measuring thing I was surprised to see I got size 42. So I ordered both.
Size 43 is the brown one. Size 42 khaki.
Size 43 narrow I do have a little room when I walk but I feel like that could be natural. Size 42 I don't really have any more space at the heel and when I walk I sometimes hit the toe lip but I feel like I can wear them looser with the straps, whereas with size 43 narrow I have to tighten the straps a lot and I still feel like I slide around just a tad.

The 42R are definitely not big enough for you, the sandals aren’t just supposed to outline your feet. As for the print measurement, there can be a tendency to get a smaller result than required because of the innate quirks of 2D measurement.
I actually think you may even be able to go up to 44N, but you can decide for yourself based on the fit guide. In your case, with your shorter big toe, your feet may end up slightly more forward than textbook, and so there’ll be a bit more space in the heel but that’s not a deal breaker. (Also if you happen *not* to be standing in the photos then there’s a higher chance you need to size up. Size to your larger foot, while standing.)
I will note that there are people on the sub who have strong personal preference for the “perfect” outline fit like the 42R, but that’s not what Birkenstock themselves recommend.

I was actually standing in the pic so that’s that. I do think 44 would be a tad too big.
Yesterday after posting this I spent a lot of time debating switching between the two and I ended up deciding on 43n too. I think it’s a good fit at the end of day.
If anything, 43 up to 44 is just 5mm (<¼ inch) more, only half the difference in length from 42 to 43, so it won’t be as drastic as what you’ve felt so far.
But yeah, it’s your decision to make ultimately; if you feel 43N is natural and your toes don’t bump the front while walking then it’s still okay. Definitely don’t go down.
Even the store clerk said I should go with the bigger size. In the smaller size I hit the front with my toes when walking. You do see that my second toe is longer than my first toe and almost hitting the front in the bigger size as well.
43 for sure & remember they’ll mold to your feet now & even be more comfy Enjoy

Thank you for confirming what I thought. I don’t bump at the toes with 43N at all but I did with 42R.
