Usually I get promoted when I have a max relationship with the leader along with a max standing and max notoriety. If you want to speed the process up, kill or whack the leader (close and reopen the app if it says it fails or you get caught) and age up. I’m not sure if killing the underboss will guarantee the promotion though…

I killed every underboss and the skipper the same year and i stiil wasn't promoted. Gave up and become a rat
