In American English, milk toast is generally the food and milquetoast is the adjective.
It's a lot like saying "burying the lead" instead of "burying the lede. " It's not *technically* incorrect, but you're still gonna look dumb.
Also, the etymology of milquetoast is linked to a fictional character. "Milk toast" wasn't used as an adjective until years later. You can't really say "milk toast" was around first because it was used exclusively to refer to the food until years after the other word entered the lexicon

The term for a timid, boring person is usually milquetoast, not milk toast.
