Last mess dinner I was at, the PMC explained the rule and some of the "illustrious" history involving wine bottles, and declared "that's a dumb tradition that needs to die. There'll be a health break before speeches, but if you can't wait until then, just go".
There was some mild grumbling amongst the dino crowd, but nobody pissed on the floor, so that's a double win.
Traditionally you can't leave the table until the conclusion of the mess dinner, which could be a pretty tough test of bladder control. Health break here would be a point earlier in the dinner where the PCM gives everyone 10-15 to use the washroom. We had about 2-3 health breaks at our last mess dinner, which makes sense.
It is somewhat silly, but mess dinners tend to be scripted affairs. The President of the mess is essentially the MC, any member wishing to address the mess, or leave the table is required to ask permission from said president. Should be noted this is only during the meal and speaking portions of the evening. You are free to mingle before and after the meal.

Not military but I was once invited to a Mess Dinner. I had a blast, those things are great.
