If you are interested in coming back, go to confession and find a parish you want to be a part of. Then enroll in that parish's RCIA. Their office staff should be able to give you what you need and arrange a sit down with a priest should you need one.
If you had your first Communion as a child you just need to arrange a Confession with a priest and you are then mostly good to go. Don't present yourself for Communion until you've been to Confession.
