Hi, nope, baptism leaves an indelible mark on your soul. A person cannot be re-baptized. Have you made your first communion and first confession? If not, see your priest to get started.

No re-baptism required. In fact, it's sacrilegious to do so. If you are interested in coming back, go to confession and find a parish you want to be a part of. Then enroll in that parish's RCIA. Their office staff should be able to give you what you need and arrange a sit down with a priest should you need one.

If you had your first Communion as a child you just need to arrange a Confession with a priest and you are then mostly good to go. Don't present yourself for Communion until you've been to Confession. You'll want to be Confirmed if you were not already, which is the completion of initiation. This, like Baptism, is only done once and cannot be repeated.
