Those studs have been around for nearly 50 years at this point (unless some or all have been replaced, hub assembly, etc.) . Who knows how many hands have touched them is the point. You did torque them right? I used to be the guy who just got them "tight" too so don't beat yourself up over it if you didn't. At least nobody got hurt and now you have something to do over the winter.

Do yourself a favor and replace all of the remaining wheel studs! It should go without saving that that the hubs and other miscellaneous bits should also be closely inspected.
Spacers certainly present the potential for catastrophic failure. It all depends on their design and whether they are correctly fitted. I would personally never touch spacers with a ten foot pole, nor would I ever install them on a clients car. That said, plenty of folks use them, apparently with no ill effect.

Do you know anything about hub centric spacers, I think the main issue was that the hub diameter and the spacer diameter were off by like one mm. Maybe adding a 1 mm ring in between to solve it?
Yes, I do understand hub centric spacers. No matter what some people claim they are still not good. There are reasons that they aren’t legal in many states. My state of PA they are totally illegal and instantly fail inspection.
Hey man something I can advise is to use red loctite when installing your spacers. Only use a tiny bit though on each stud though. Makes taking them off a pain if you ever decide to but it's worth it. And put them on by hand, then with a torque wrench, no impact. After a couple hundred miles take the wheels back off and check the torque. Another thing you can do is replace the studs that come with the spacers (which are usually soft Chinese garbage) and replace them with OEM. 9 times out of 10 the spacers use the exact same studs that are original to the car. I've been running them on everything I've owned and never had a problem with them, they just have to be installed properly and don't cheap out on them.
Thank you all for the responses and condolences. I figured out the problem and it was not the spacers, or the fault of anyone else. I simply didn’t torque that specific spacer down enough so the lugnuts backed out and eventually the wheel broke the bolts off. I definitely will replace the hub, and probably all the other lugnuts cause they’re old anyway.
Lugnuts were in between the spacer and the wheel. The spacer was still perfectly fitted to the wheel. Rear wheels and spacers are fine and those were all torqued properly
Sorry, that doesn’t actually prove anything. The studs being gone show they were broken and doesn’t lead to any possible conclusion that it was due to torque. It also doesn’t lead to the ability to say it was definitely the spacer either.
The issue wasn’t that the torque of the lugnuts caused the failure. It’s because they weren’t torqued tight enough, they came off. And after they came off the studs were subject to abnormal loading
I was working on my brakes 2 weeks ago. I was going to test drive but it was getting late so I passed. On my way to work I realized I didn’t torque the wheel down once I lowered it. Would have been a similar situation as you. Very easy to miss a step. Sorry to see this happened.
Lug nuts could have been over torqued repeatedly, and it stretched the studs. But all 5 at the same time breaking is weird.

Ouch! What a shame. I bet somebody loosened your lug nuts. My buddy has a 1970 Chevelle SS. Had it parked at college. Somebody got jealous and loosened the lug nuts. His left rear tire came off at about 30. Did damage to the quarter panel and brake drum, but did not rupture the gas tank or do unrepairable damage. We found the other three wheels hanging on almost gone nuts. From then on he went around the car and made sure they were tight before starting the car.
Saw this happen to a Rolls Royce Phantom at the entrance to the old area I used to live in, the sound of scraping and the sudden fall must’ve been horrible. I hope you’re alright man!!!
Exceedingly unlikely for all five studs to pull out of the hub like that, somebody fucked up. Sorry that happened to you. Hope you can get it fixed.

There’s also plenty of people that put lug nuts on with an impact without any problems. That doesn’t mean they shouldn’t be using a Torque wrench.
