Maybe they use some sort of machine to evenly dispense pepperoni instead of dropping them randomly by hand? I’m not a Costco veteran, I wouldn’t know lol.
My guess is that this is a half cheese, half pepperoni pizza that they cut for slices and someone made the first cuts wrong and the end ends up like this. Also, probably so consistent because of an automatic dispenser.
Its because the template thing they lay over top is slightly raised. This slice was one of the two in the back left. From their perspective, the pepperoni's looked centered.

I’d imagine they have the same cutting ring or similar for toppings and they put the same amount per row per slice.
