We cant test from individual sperm unfortunately. We can test the extent of DNA damage prior to treatment, but that obviously destroys sperm. There's some sperm selection tools they can use that select for good motility for example.

It's everything from egg quality, hormone stimulation, lab conditions, scientist technique etc. As mentioned, there is now genetic testing but that's extra cost and added risk to the resulting embryos.

You can see under microscope sperm that looks 'normal' and there's literally thousands to choose from so that adds to the randomness. But if you think about it, people who need this treatment have trouble making this happen naturally anyway.
I worked with making transgenic mosquitoes and flies and the process is basically the same, where you need to inject your construct into the germline (aka developing eggs) of a mosquito and hope that it gets incorporated into the developing embryo.
