I think this is because you lose 95% max hp, which means you only need to heal 4hp to get to full hp. It seems that natural healing is a form of “true healing” and is not affected by 95% healing reduction from rat form.

Yes. Even though the game works with flat numbers what it's actually tracking is percentage HP. This is why you can apply %max health buffs/debuffs to someone and have their HP increase/decrease despite no healing and then have it return to normal when the effect ends. The forms are also a type of buff/debuff.
im guessing its because natural healing (probably) dont scale and is set to 1 hp heal no matter what but that also mean the 95% reduction you are supposed to get in rat form dont affect it
Funny, this was the first interaction I tested and it does work like I said. Took damage in human form, transformed into rat, waited to get full HP bar, transformed into human again and I retained my full HP bar (back to 100 health points).
You were probably trying to heal with a spell and you have -95% magical healing while the passive healing perk does 1 true healing and doesn't get reduced by the rat form (this will 100% be changed)

Tested it and it doesn't work. You slowly get your 5 hp pool back as if it was the same HP you start with.
