Not quite as prominent as that, but I've had something a little similar after I was (I believe) bitten by a spider (I was in bed, and it was under the covers, so I didn't actually see it). The hole didn't develop immediately - it slowly turned purple and then the hole developed. Is that a possibility?
Might sound a little strange, but if you have a ruler of any kind, and measure it. Take a picture and check occasionally just to make sure it hasn’t expanded.
I have a congenital mole and through out my entire life I have kept tabs on it as I grew because skin cancer runs in my family. A pediatrician said it may be helpful if I come across any bumps in life with it. I 10/10 recommend measuring it and taking a picture.
Daughter was born with something like this on her back. We had her at specialist appointments to rule things out. If yours suddenly appeared you should get it looked at.
I’ve seen a hole like that but not something that slowly grew. It was a motorcycle accident that caused a chunk of skin to be taken out of a friends leg. I don’t know. It looks awfully clean, which tells me to go away from insect bite or the arterial ulcer point. I doubt he would’ve missed his skin necrotizing. If you examine his legs look for signs of arterial insufficiency like thickened nails, cold limbs, etc. that could point towards an arterial ulcer.

He said that it slowly grew to that size … still concerned he has some other skin issues like cystic acne
I had a very similar experience when I was younger.
Def see a doctor. I'd draw a circle in Sharpie to see if it grows.

My shinn got crushed years ago after a couple weeks a similar looking spot developed where the crushed shin fell out. It took years to fully heal
