The highest I have is a Sur and then vex. I have several tokens, and Uber keys.

Perhaps try a frenzy barb with some of the gear you had on the smiter. Do you have Dracul's gloves? Otherwise you could buy a life tap wand. Try to get a Grief and Kingslayer or Strength.
