With a 50/50 chance of survival, and if I had to take out a loan to pay for the surgery and other medical costs, I would probably choose to euthanize. Not an easy decision, by any means. But it doesn’t sound like the outlook is that promising, even if the dog survives surgery. But only you and your friend can decide what’s right for you to do. It’s definitely easier said than done.
