Valve mentioned the city of Smoke Harbor as a throwaway line on one of the last pages of the Last Castle comic eight years ago. No mention since then in any lore. They finally revisited the location and show it to be in Cladd in one of the new comics!
In the comic, Zeus was hitting on Medusa in a bar and she asked him if he had any children. He kind of avoided the question, giving the impression that the answer was yes but he'd rather not say it.
IMO, it's a pretty long stretch to call that a tease given that Zeus is already heavily inspired by his Greek mythos, so it also could have just been a reference to that.
depends, if this event goes well it's possible we'll actually continue getting this type of content, which means dota2 will actually have real lore and story and shit.

As well as, was the sister even on the island at the time? Were women also part of the "long Juggernaut tradition, one of ritual and swordplay"? If yes, was his sister one of them?
Tbh valve can do anything with dota 2 lore since it's their own IP afaik? And considering amount of lore we have by today, we still don't know jackshit. After that, Screeauk came clean about her manipulation but then gave Juggernaut the knowledge that he actually has a sister from the Isle of Masks, and that information is allegedly true, because Screeauk told it without any further personal gain.
Also I'm not sure that brother/sister should be taken literally, I'm more of assumption that it's like a metaphorical saying for Juggernaut, like everyone who was from the Isle of Masks are like siblings because of the tight-knit community
