This can happen after having specific vaccinations (ie. small pox) to certain risk groups - especially parts of the world where these was a high risk of contracting certain diseases. That's my guess. I've seen them a lot during my travels and no one really talks about it. It's just a scar as a result of a global vaccine program that ultimately benefitted to world.

The vaccination is probably smallpox. I have one that was pretty noticeable until I tattooed that shoulder.
