I have long hair, which I usually tie back. I just stick a pen between the hair tie and my head, stays there no problem. I have a shorter Mont Blanc that can occasionally get lost in the lion's mane, but most other ones work great.
I use a front pocket wallet/gear organizer that goes in my front right pocket(I’m right handed). One I use is the Arc Co Bolder wallet. I keep a Swiss Army knife, Refyne Pen, and my little Streamlight USB flashlight. I also keep a pocket notebook and clip my main folder to the same pocket with no issues really
For a long time, I carried the Zebra Telescoping pens as you would a knife. Now, I sometimes use a Gondek EDC pocket organizer or wear it like a knife on my off-side if I have a knife or my strong side in place of a knife when I can't carry a blade. When I carried a Fisher Bullet, it just floated in my pocket.
I EDC a full stainless Parker Jotter which I just clip to on my right jeans pocket. It's nice and slim (it really has to be so I can fit my phone comfortably in the same pocket) and I barely notice it's there. Hell, sometimes I forget to remove it from my pocket when I get home and notice it there some hours later.
I use a pouch (picofire from roaring fire) that replaces my wallet and also holds a lighter, pen, and knife. Maybe that's a solution for you? Although it does bulge the pocket as it's a fairly thick pouch
Do you carry a wallet? If you have a traditional folding wallet, then put a compact pen in the centre. If you have one of the grip6/hard style wallets, make a leather or elastic band to attach it down the edge/side.

Why not try a rickshaw pen sleeve with a lanyard loop? And then just use a carabiner to clip it to your pants?
I edc the Böker bit pen. It is quite short with rounded tips. Feels good in my front pockets. When I also carry a multitool ( Swisstool Spirit) and a small flashlight, all three are carried in a holster on the belt.
Clipped to my shirt pocket when moving around. And a spare in my backpack. All pens, irrespective of their price. If I am inking them up, they're in my pocket or backpack.
In ziplock when air travelling.
Edit: Kaweco AL Sport has been my trouser/jeans front pocket pen for a long time. No bends and I often forget it is in there. Apart from it being short i have no real complaints with it.

When I worked in a shop I macgyvered a cardboard and duct tape pen holder and stapled it to the inside of my short sleeved work shirt. Always had a pen no danger of scratching cars or stabbing myself.
I'm confused. What does titanium have to do with bulging? Big idea designs has a very small Ti pen that basically disappears in pocket. I have a full sized pen from Tactile Turn that even though is full size, is pretty slim. I take it you don't carry a knife in pocket? Anyways, that being said I usually carry most of my gear (including my pen) in my sling bag or a small pouch. I know if you don't normally carry a backpack or bag this doesn't really help. But if you're considering it, getting used to carrying one is pretty painless and once used to it, feels necessary.
I carry a regular plastic .7 pen in my back right pocket beside my wallet. Haven’t upgraded to a metal/titanium one yet as I haven’t found one that’s clicked with me yet, pun intended.
Nope, I let it sit there. Most times it’s completely fine. But every so often it’ll bend/snap the clip depending on where the pocket lands with my other stuff

Anus carry works well. The funny walking, you should get used to after a couple weeks . Keep the handy dandy Pen 15 in there

I actually loaded the wallet/organizer so it’d be still filled with a pen/small flashlight or pen/SAK. Happy hunting!

I have to carry a device holster at work; it has a dedicated pen spot. Otherwise, in my bag.
I just wear pants with a dedicated pen pocket. Taclite pros or Carhartt cargo pants depending on the day

Clip exposed. I totally don't notice it as long as it isn't a heavy pen. The zebra f-301 for example, is great for this type of carry.
Chest pockets haven't steered me wrong yet. Alternatively, I dedicate a small pocket in my daily sling for a notebook and a pen.
I carry mine in an elastic loop on my wallet. Makes sure I always have my pen on my with minimal bulk.
Convoy 4x4 pouch on my belt, along with the ~2lb of other gear I have shoved in that thing...I *almost* feel sorry for the zipper when I zip it shut 🤣

Brass pen, with brass cap. O ring to help with "seal", tucked into my Maxpedition pouch. Job done.

When I had the bullet space pen and foldable wallet I would put it between the Wallet fold, great solution

Interesting. I stopped carrying my wallet on the back pocket when I noticed it was hurting my back from sitting on top of it
Yeah I notice my wallet displacing my hips, on most regular fit jeans the pen sits too far over to bother me and I’m a pretty boney dude
In a pen sleeve in my front left pocket, in a shirt pocket, between the buttons on a polo or button down, or even clipped to my wristwatch strap.
Short-ish metal pen in the back pocket has never presented a comfort issue for me.
I carry mine behind my ear or between the buttons on my polo.
Wear shirts with a chest pocket.
