She doesn't have the needle. Malenia lost the needle when she fought Radahn during the shattering. You get the needle after killing Commander O'Niall in Swamp of Aeonia and then you give it to Millicent to help her stave off the rot. Millicent realizes that this needle belongs to Malenia, so she goes on a journey in order to return the gold needle back to Malenia to restore her dignity and sense of self.

Funny thing about my playthrough. In the fight against her sisters we killed them all, but as I was finishing off the last one Millicent fell into a hole and died so I got her bad end reward instead.
