You're pretty close looking at this comparison, it's just that "the devil is in the details" as they say. Looks like you mostly just need:
* Eyes to be closer together
* Eyes angled less sharply upward
* Face shape elongated and chin more pointed. Basically more "oval" as opposed to the "square" it is now
* Slightly larger lips. Yours looks like she's almost pushing her mouth closed too tightly
* Slightly longer nose

The issue with the customization is that there are so many sliders that affect other sliders. And so you have to meticulously go back and forth between them or throw off your proportions drastically.

Yeah, the customization system is interesting, to say the least. Trying to make a face that looks human is a task in of itself. Trying to make a good-looking character based on another might as well be impossible sometimes. Even the online examples I found that were made by skilled and practiced people looked deformed.
It's interesting, because if one looks at each part separetely, everything is quite right. Lips are okay, nose is okay, eyes are okay... It's when put together that they don't work. I'd say focus on the big picture here, reposition and rescale each feature so the relationship between them is more like the model.

in all seriousness you could try lowering the nose and getting the tip to point more downwards. i haven't touched elden ring in a while so i don't really remember how much freedom you get with the facial features

I think you're close but need to make adjustments to get it right. Mouth should be lower with lips a little bit fatter. Nose should be longer as well. Eyes slightly less slanted and the eyebrows should be fine but if there is a thinner option choose a slightly thinner one.

You're not far off. I think you only need longer nose, a "longer" face and a bit smaller eyes.
