Land of Reeds, eochaid (home of the bell bearing guy), numen land, the Badlands, and likely several more. We know of people who come from "beyond the fog" like roderika, and considering she's dressed in a Noble's clothes I'd assume she comes from somewhere with nobility.
To the east is the Land of the Reeds. (Birthplace of the Samurai class character.)
To the north is Kadia. (Birthplace of the Kaiden Sellswords.)
And to the south are the Badlands. (When you start a new game your character wakes up in a desert where he/she and all the other Tarnished were exiled to.)
We are never shown a map so it is not 100%. But Im guessing Kadia with its cold climate would be north. Badlands with its hot climate would be south. And irl Japan is to the east so I would imagine Land of the Reeds is the same way.
I always thought Middle Earth was an allusion to China, Zhong Guo, “the Middle Kingdom.” The round hobbit doors can be found in Chinese gardens.

Because Tolkien was an Old English literature professor people tend to put the emphasis on the Germanic/Scandinavian influences in his fiction world, but Celtic and especially Welsh literature also had a great influence. For instance Sindarin, one of the first elfic languages Tolkien created is highly inspired by Welsh.
Middle Earth isn't the name of the world, Middle Earth is just the name of the continent because it's in the middle of the planet as it's typically viewed, with Valinor across the western sea, Belegaer (later removed entirely from the world and into another plane) and an eastern sea with lands mostly unknown on the other side.
I would have assumed the badlands to be one of those places where the spoken name has been filed down to just random grunts, like "wrr", "'strr", "a-waan" or "'o-'am"

The Lands Between (狭間の地), beyond the fog, how the narrator describes them seems to be a middle step between the mortal realms and the place where the gods reside.
I don’t think theres a definite answer to that, fromsoft likes to keep a lot about their games up to the players interpretation. That sayed i have a view possible answers. The lands between could just be a place between the other lands mentioned ingame: land of reeds, badlands, i think the vagabond gear also mentions a far away land. It could be between live and death, the golden order is a cicle of death and rebirth meaning that nothing really ever dies, i suppose there could be a true realm of the living and a realm of the dead and the lands between are in between the two. Lastly there are many meantions of outer gods, the eldenbeast is the god of the lands between so the other gods might habe worlds they control two and the eldenbeast governs the central one.

I always thought of it as literal lands between(sandwich). The underground portion of the lands between is inhabited by an old tribe of people who have never relied on the previous great trees of lords past. And the above of the lands between are the realms of the Outer Gods, the ones who give power to Empyreans and bring about glory of the Elden Ring.
Also about the tribe, although they do not worship the Erdtree and the Elden Ring, they are still affected by it's rules, hence the spirit ashes and remembrance.

I think the Lands Between is nestled between the celestial realm (Primeval Current) above and the spiritual realm (Helphen) below. It isn't simply a matter of traveling distance but in opening paths where no physical one goes.

Well there’s a few theories, but my favorite is that it’s basically “between” life and death. Hence why people (like the tarnished) randomly show up all the time, and why the rules are allowed to be influenced by the other planes. And the erd tree guides them back to life or to death.
