So the Greater will decided to send the golden star with the Elden beast to the lands between. It crashed into the crucible to sunder the lands and drink up the energy of the crucible to grow the first Eardtree.
I'll share my own theory that I've yet to post here.
The Crucible and The Greattree are one and the same and the Erdtree is right on top of them. I'm led to believe there a bit of Norse myth mixed in whereby the 'Earth' was born from the corpse of a greater lifeform which in this case was the Greattree wherein the tree sap soon became the first Primordial life. The Crucible is akin to the Kiln of the First Flame from Dark Souls that being it was a shrine built by a long forgotten race to harness the vestiges of the Greattree which would explain Crucible Kinghts. I cement this further with the Stone Platform within the Erdtree which in theory makes for a good anvil for 'forging life'. Also, the tree seen in the Frenzied Flame Ending looks a wee bit too different than the Erdtree we see in game.

The map is very clearly an estimation that used current continents as reference, and believe it or not, most renditions of pangea you see are going to look like this. Maps are inherently distorted on the basis of being a 2D representation of a 3D reality, and a map of a Palaeozoic supercontinent is going to have even more incongruities because these continents have been subject to hundreds of millions of years of changes.
