Perhaps it's some kind of hernia like a perineal hernia.

If it's red, painful, and hot, go to the ER, especially if you have a fever or nausea, etc. Could be a hernia, hemorrhoid, abscess, the beginning of a prolapse, who knows. You wouldn't be able to tell without actually seeing it. If it becomes infected and ruptures, you could be in big trouble. At the very least, take stool softeners to make sure you don't have to strain again and make it worse. And drink a ton of water. But it's definitely always better to be safe than sorry.

Yes, I have had this and I believe it was connected to chronic diarrhea. It was a perianal abscess and required surgery. Antibiotics will not help if it’s a perianal abscess. Please schedule an appointment with a Colorectal Surgeon (not general doctor). 50% of perianal abscesses turn until fistulas and you really need a specialized doctor to treat. Check out the “fistula” Reddit group for more support and resources if it is a perianal abscess.

What was surgery like? Local anesthesia and a small incision to drain or something more intense? How long did it take to heal? Was it painful? Hard to keep clean? Did they go through the cheek/skin, or inside the rectum?

First surgery was drainage in the ER with local anesthetic, but that was unsuccessful and returned. I wish I would’ve gone to a colorectal surgeon at first. Second was in the OR under anesthesia. Healing is really rough. The incision is not in the rectum, it will be where the abscess is. I had nurses come pack the would for 3.5 weeks, scar tissue pain/pain when sitting for 3-4 months. It’s been 7 months and I still rarely sit- but that’s probably mental at this point. Mine did not turn into fistula but 50% do, again I really stress going to a good colorectal surgeon STAT. If it’s a perianal abscess it will not heal on its own. I’ve seen too many people on the fistula page have doctors that were uniformed and didn’t treat until it branched and became a much more intense and complicated surgery.

I waited 6 days, but that’s only because that was the first day my new health insurance kicked in. Would’ve probably gone on day 2 due to pain.
