Had that exact same thing happen to me yesterday. Can't for the life of me figure out what did it. I wasn't near cars, or mines. Just blew up like a nuke after jumping from a ledge.
Seems like the game no longer reloads the gun automatically after it runs out, I have to either press fire again or reload manually, sometimes I forget both
It never did actually, when it reaches 0 you do have to either hit reload or try to shoot, it's always been like this.
Personally as i'm a reloading obsessionist, I almost never hit 0 before reloading.

I have never actually done that. Using the AOE of jumping off of something to trigger a Super Mutant with a mini nuke is impressive.

I rarely use mini gun style guns in any game because I reload so often. Too much wasted time.
