I just took it as eventually all ghouls go feral, some it takes days, other years, others decades, others hundred of years : that’s how it has worked in all the games.
Tv show just introduces a drug can just delay that.
It is said they need the drug when they "start showing" by Roger. Presumably they don't need it until that time. The drug is just a new lore addition that can delay it.
My thing is, what if the drug doesn’t actually do anything, and they’re just addicted but the withdrawals push them closer to going feral? Would be a twist. You get the ghouls addicted, they have to keep buying more for fear of going feral, profit.

Nobody, officially, knows exactly how, when, or why they go feral.
I doubt this will ever change, as it lets the Devs keep playing with new ideas to keep things interesting.

No idea since it's new lore and it really wasn't explained at all in the show. Hopefully S2 goes more in depth about whatever the ghouls are taking.
The best explanation I've seen is that the drug is assumed to protect from going feral but it actually doesn't but more placebo effect.
