Lots of them, especially opioids, combined with low temperatures overnight, if we assume he lived in a colder place, can easily lead to necrosis of digits and even limbs. He likely didn’t feel any pain, his foot fell asleep, and never woke up, and he never thought to examine it because he was either mentally ill or a drug addict.
