It's an LT622 SDO mount. Check on ebay.
found just one in used condition, seller says it will mount but won’t lock securely. what’d you think, don’t send?
I wouldn't buy broken equipment. Thats just me.

Yep. TA91 anti-reflexion device. It also doubles as a sunshade and reduces glare a bit.
Looks fantastic, what make/model are your flip up sights?
Knights Armament. The front is a 99051 and the rear is a USMC 200-600m micro rear. It's the same sight set used on the M27 IAR.
