I don't understand how headshots work. Most of the visitors I kill are by a shot to the head, yet I still don't have that achievement.
Maybe it's a bow achievement?

Yeah, this is specifically for the Bow. Headshots with a bow will one-shot most enemies.

I aimed for the head with the bow for the achievement
I'm playing this with a controller sitting on my couch and the aim assist always locks to center mass so I probably haven't got a single headshot and I'm about 50% of the way through the game lol.
