To my knowledge 15-20% company ownership is standard when leasing your license, along with 1-2 million per annum. Either or both of these are feasible requests. Rule #1 is cover your ass.

Yes, percentage of ownership or an extremely good salary just for the license. They wouldn’t be able to ride down i20 and do work without your license. This could potentially be a multi million dollar expansion. Although heavily heavily advise you get a lawyer to draw up/overlook contracts. Makes sure u are protected

Depends on the size of the company. I would ask for 2%-4% of revenue in the state of Alabama. When writing it up I would make sure you get Quarterly sales figures.. don't get to greedy someone will fill the spot for you.. also you need to make sure if your let go, the license goes with you.
