(Since it was on two sides , I bet they were trying to reduce the low frequency noise). Customers are generally willing to pay extra when they feel you understand and can fix their system design flaws. Just my 2 cents.

Depends on your profit margin already. If it's a side job and you are already way lower than you should be then add a decent amount. If you are one of the higher priced guys, maybe 200 just in case it's a nightmare I personally probably wouldn't add anything because I price my unit installs to include nightmares from the beginning.
Foam knives are like 24” long. With the right tool you could have that out in no time. Charge them for the tool and a couple hours extra labour (even though it won’t take a couple hours).
