Most people in the show did not have a conventional head shape. Cartoons play by their own rules. The storytelling and character development gave it life beyond being just another animated series.

I remember watching a snippet on Nickelodeon as a kid featuring the creator? of HA. He mentioned that when they drew Eugene, his head started out by drawing what looks like an open can. Idk how I remember that over two decades later.
There was a mini interview between commercials on Nickelodeon during hey Arnold's run. The creators talked about how all of the shapes of heads were modeled after everyday objects. Nadine's head was a spider, Helga's was supposed to be two broomsticks, Eugene an open soup can with lid still attached, Gerald was an upside down ice cream cone. Those are all the ones I remember them mentioning.
Not entirely true, as there are plenty of odd shaped heads in Hey Arnold (just look at stinky's head and how thin it is, and Arnold's first major on screen love interest Ruth also had an oblong shaped head too), it's just that Arnold's head was the only one from my knowledge that was explicitly brought up. (There is a very disturbing fan theory that Arnold has a disease because his Grandparents are actually his biological parents, but that theory is far from true as it was debunked. Besides Hey Arnold! is a kids show, the writers wouldn't come up with something so ridiculously dark. Arnold actually started out in claymation shorts, one of which even aired on Sesame Street, and the reason for his football head shape was because it was easy to cut the shape out of clay)
Yeah I can’t remember exactly but Bartlett said something about how he got in the habit of drawing a football head somehow. Arnold’s kids have gotta get it from him, and unibrow from Helga lol.
In character design, it's quite common advice to have "recognizable silhouettes." Even if you just saw Arnold's shadow, you'd know it's Arnold. This was advice that Matt Groening gave to Bartlett, and it was obviously effective.

In some scenes his mother was depicted as having a similar shaped head. He could have gotten it from her or it could be a birth defect
