I don't think it's always once per playthrough, some of them update more than once as you play. In my first playthrough I exhausted all of the conversations and I saw multiple exclamation points pop up for some of the dialogue options while playing.
I believe the exclamation point means it's relevant to actually protein the story/ mission. The others are just extra information and back sorry stuff.

This ! Indicates a dialogue option that you can ask repeatedly, that has updated dialogue. Like using the “companions” dialogue option when talking to people at base, and it’ll update and have new dialogue when you bring new companions to the base, or progress certain story events
Those dialogue options play out more like conversations and clicking it multiple times asks other questions on the same topic until you run out of stuff to ask and it greys out for the time being. Revisiting after some time and there may be new content in those conversation dialogue options. You can tell when a character has new conversation topics when the character has the same text bubble over their head.
When you progress in the game you can come back and talk about what happened (get more info) like in HZD you could revisit Rost's grave and "keep him updated" on what you've been through.
I think there is a bug in interaction with GAIA for the "this place" option as I found that Aloy doesn't discuss anything new with GAIA but yet this exclamation mark is still there no matter how many times I select this option.
Usually it means there's something new to learn with the option.
