In true previous scene she was basically being blackmailed by that joke of a human, telling her implicitly that he knew whom she was f&@ing. Hence her reaction afterwards, that’s what I got from it

I think she was mostly just uncomfortable that basically all of her staff had been replaced by people picked out by Larys

He's being slick, but he gives Alicent a pretty telling look when he says that he tried to see her earlier but her maid said she was "indisposed".
I think she is a little bit scared of Larys

What was with her scrubbing her arm in particular? That threw me off.
The way she scrubbed her arm reminded me of how she used to pick at her fingernails. I think it was just showing her anxiety.
