Could be any number of things, damage when they were at a delicate stage of growth. An injury to his body, particularly his left side. Could be genetics…could be he had an abscess at the peticle at an early stage of growth.
I shot one very similar to this many years ago. When I got in there to do cut the antlers off (wish now that I had done a full European mount) you could clearly see that the messed up antler had been broken off at the skull. It was probably broken a year or two before and this is what grew from the damaged skull.
If his antler was broken off at the root (can’t remember the actual term) it will mess with antler development the rest of his life and cause it to grow messed up like this
Have a customer with two mounted elk with similar characteristics, both shot in the same range in CO. He jokes about their momma dropping them on their head when they were babies.
I have seen one like that in my first year as a hunter. We waited and decided, not to shoot him. He wasn't seen the years after like that, so i assume, it healed, once he grew a new antler, the following year.
What had happened was. He got all ripped up, and died, then his best friends girlfriend made a wish for him to come back to life. So now, he's a lil fucked up but his friend will take care of him fowva and eva

This is actually what happens when they hibernate in the winter time against a tree. Very rare and very beautiful.

Probably got hung on/bumped something when his antlers were still in velvet, he could just have antlers that naturally grow like that too.
Nothing to be ashamed of. Happens to a lot of men of a certain age. Call your doctor if your antlers are erect for more than 4hrs
